Have cetaceans evolved from animals that were always in the water?

No.  In fact, fossil records show that cetaceans evolved from land mammals, particularly cloven hoofed animals that had unusual traits that are shared with the earliest cetaceans.  The closest modern day land relative to the cetacean is the hippopotamus.
