What are the risks involved with mixed gas diving?

There are a few risks when it comes to mixed gas diving.  One is improperly mixing the gas and another is the risk of fire or explosion due to the use of pure oxygen.  When diving with mixed gases, there is a risk of oxygen or carbon dioxide toxicity or nitrogen narcosis, which depends on the mixture of the gases.
