What are the risks involved with altitude diving?

When diving in general it is important to pay close attention to pressure and to ensure that your ascent rate is appropriate for the depth you have been diving.  When diving at altitude, the pressures are different and this means you have an increased risk of the bends, should you ascend too quickly.  With increased altitude there are also concerns of potential hypothermia and hypoxia.
